Student Organization Renewal Form
Step 1: Complete Paper Application for Student Organization Recognition & RenewalIn order to complete the renewal process, you must complete and return an up-to-date Application for Student Organization Recognition & Renewal Form. You can print one online here or if you'd like a hard copy, visit the Office of Student Life.
You must have five different students listed on the form. Officer's titles are not necessary at the time of your renewal appointment unless you have them. For those organizations who have not selected officers, an update will be expected in the fall to the Office of Student Life. Any services that you check off on this form will be discussed between the president and/or co-leaders and the Student Activities Manager before access is granted.
All officers/contact persons for the organization must sign the above Recognition & Renewal form. By signing, the officers/contact persons ensure that they agree to uphold all university policies and procedures, to not use student activity money for political activities intended to influence voting on issues or candidates currently on a ballot, nor to participate in hazing activities. It also grants release of eligibility status information to the president and advisor of the organization by the Office of Student Life. In addition, all organizations must have a Constitution/Bylaws on file with the Office of Student Life; we encourage organizations to update constitutions every two to three years.
By requesting application for recognition or renewal, the officers/contact persons agree to abide by the rules and regulations of the university, specifically those which regulate student organizations, so that they may continue to use the name and facilities of Kent State University at Stark. The organization acknowledges that every voting member of this organization is a registered student or member of the faculty or staff of Kent State University.
